HOURS:

Open Monday through Friday 8:00am to 430pm

Lunch is taken daily from 12:00 noon to 1:00pm

 

INSURANCE:

Accepting Kentucky Medicaid and Managed Care Organizations, all private commercial insurances. Currently awaiting to be credentialed with Tricare. 

 

PAYMENT:

Cash and all major debit and credit cards.
